A headache during sexual activity may be a primary headache associated with sexual activity, in which no intracranial disorder is found. That diagnosis can be made only after dangerous causes have been excluded. A first sudden, severe “thunderclap” headache requires stopping activity and going to an emergency department immediately.
What Can It Feel Like?
Pain may begin as a dull ache in the head or neck and intensify with arousal, or it may become explosive just before or at orgasm. A primary attack can last from minutes to hours. Symptoms alone cannot reliably distinguish a primary headache from bleeding or another vascular disorder.
When Is It an Emergency?
Seek emergency care for a first sudden severe headache, pain reaching maximum intensity within seconds to a minute, or pain accompanied by loss of consciousness, confusion, seizure, stiff neck, repeated vomiting, visual change, weakness, numbness, speech difficulty, or chest pain. Repeated explosive headaches over days or weeks can be associated with reversible cerebral vasoconstriction syndrome (RCVS) and also require urgent assessment.
Why Must the First Attack Be Assessed?
Clinicians must exclude subarachnoid haemorrhage, head or neck arterial dissection, RCVS, and other secondary causes. Evaluation may use CT/CTA, MRI/MRA, lumbar puncture, or other tests depending on the timing, symptoms, and initial findings. No single scan is appropriate for every person.
When Is a Primary Headache Diagnosed?
ICHD-3 criteria apply when headache is brought on by sexual activity and no intracranial disorder explains it. A single episode may be classified as probable primary headache, but serious causes still have to be excluded. Loss of consciousness, vomiting, or visual, sensory, or motor symptoms are not typical features of the primary disorder.
Care After Serious Causes Are Excluded
Stop activity when pain begins and return gradually according to medical advice. Record timing, severity, associated symptoms, medicines, and the activity involved. A clinician may consider preventive treatment or medicine before activity for recurrent attacks, but the choice depends on other conditions and medicines. Do not start or stop migraine, erectile-function, or blood-pressure medicines independently.
