Lenacapavir is a long-acting HIV capsid inhibitor, but its indications and product names must be distinguished. Sunlenca is used with other antiretrovirals to treat multidrug-resistant HIV; it is not PrEP. Yeztugo is the US product name for lenacapavir as PrEP, approved in 2025. Authorization and brand names can differ by country.
PrEP Evidence
PURPOSE 1 observed no HIV infections in the lenacapavir group during the primary analysis. PURPOSE 2 recorded two infections in the lenacapavir group and an incidence about 96% below estimated background incidence. These findings show very high efficacy, but they are not a 100% individual guarantee.
Use for Prevention
The PrEP regimen uses a subcutaneous injection every 26 weeks under the authorized schedule and may include oral initiation doses according to the local label. HIV-negative status must be confirmed before starting and before each later injection. Giving lenacapavir alone during undiagnosed HIV infection can contribute to resistance. A person who is late or stops needs testing and a continuing prevention plan because drug levels remain in the body for a long time.
Side Effects and Limitations
Pain, swelling, redness, or nodules at the injection site can occur, along with other effects listed in the product information. Lenacapavir does not prevent other STIs or pregnancy. Some medicines can interact with it, so disclose all medicines and herbal products to the provider.
Access in Thailand
US approval does not mean that the product is immediately available or reimbursed in Thailand. Check current authorization with the Thai FDA, the clinic, and the relevant health scheme. Other approved PrEP options remain highly effective when used as recommended.
