Living in the same home as someone with mpox does not automatically cause infection, and there is no universal rule that infection occurs after “four hours.” Risk depends on the type and closeness of contact, especially direct contact with a rash, scabs, or body fluids; hugging or sex; close caregiving; and sharing items that have touched lesions.

How Is Mpox Transmitted?

Important routes include direct skin contact with lesions, close contact with body fluids, and shared bedding, clothing, towels, or objects contaminated by lesions. Prolonged close face-to-face contact may involve respiratory secretions, but mpox does not spread like a typical long-range airborne infection. Transmission during pregnancy can occur, and some people have a higher risk of severe illness.

Why Is a Four-Hour Threshold Misleading?

Time in the same building cannot determine risk by itself. Assessment must consider contact with lesions or contaminated objects, bed sharing, intimate contact, or direct caregiving. Briefly passing someone, staying in separate rooms, and providing hands-on care are very different exposures. Public health staff should assess what actually happened rather than applying a fixed time threshold.

Reducing Risk at Home

  • Avoid contact with lesions and scabs and avoid sex until all lesions have healed, fresh skin has formed, and a healthcare provider advises that it is safe.
  • Do not share bedding, clothes, towels, or utensils; clean and launder items according to public-health guidance.
  • Cover lesions when possible, wash hands regularly, and use a mask during necessary close contact when advised.
  • Ask promptly about post-exposure vaccination if you may be an eligible contact.

When Should You Seek Advice?

If you had close contact with someone who has mpox or develop a new rash, blisters, fever, swollen lymph nodes, or other concerning symptoms, avoid close contact and contact a healthcare service. Do not diagnose yourself from a rash image alone because several conditions can look similar.
