Lenacapavir is a long-acting antiretroviral with highly effective trial results as pre-exposure prophylaxis (PrEP). The widely reported figure of “about US$40 per person per year” is a target supply price for certain eligible low- and middle-income countries through generic-manufacturing arrangements. It is not a worldwide retail price and does not mean that everyone in every country can immediately obtain the medicine at that price.
HIV-Prevention Trial Results
Subcutaneous lenacapavir given every 26 weeks was studied in PURPOSE 1 and PURPOSE 2. No new HIV infections occurred among participants assigned lenacapavir during the primary analysis of PURPOSE 1. PURPOSE 2 recorded very few infections and an incidence significantly below the estimated background rate. These findings demonstrate very high efficacy in the study populations; they do not guarantee 100% protection for every person, exposure, or period of use.
What Does the US$40 Figure Mean?
The figure is a negotiated supply-price target for defined markets, subject to eligibility, licensing, manufacturing volume, procurement, and timing. It cannot be directly compared with a US retail list price or used to predict what a person in Thailand will pay. Actual access and cost depend on national authorization, procurement, health coverage, service fees, and product availability.
What Could It Mean for PrEP Access?
A twice-yearly injection may help people who find daily tablets difficult, reduce the burden of storing medication, and expand prevention choice. HIV testing is required before starting and before scheduled injections because using PrEP during undiagnosed HIV infection can contribute to drug resistance. Injections must be given on schedule, and anyone stopping must have a follow-up prevention plan because lenacapavir remains in the body for a prolonged period.
What Should Thailand Monitor?
- Thai regulatory authorization, indications, and clinical guidance.
- Which countries and populations qualify under supply-price agreements.
- Funding, HIV-testing systems, follow-up capacity, and clinic readiness.
- Equitable access without stigma toward any population.
Until lenacapavir is available through an authorized service, use PrEP options approved and available locally under clinical guidance. Do not buy or inject products from an unverified source.
